Three New Tracking Codes, is Canada Using Them?

News came out today regarding three new diagnostic codes: Z28.310, Z28.311 and Z28.39, that have been added to the International Classification of Diseases, Tenth Revision, Clinical Modification (ICD-10-CM), for reporting on patient COVID-19 vaccination status. 

As noted by others, diagnostic codes using the ICD system have been integrated into medical billing systems.

Z codes, according to various sources such as university health websites among others, are for tracking purposes only.

Canada has apparently had quite a role in the shaping of these codes, and brags about it on a website dedicated to the International Classification of Diseases.

typingThis website provides a PDF to help medical practitioners know how to code for covid pandemic events, including “vaxx” efforts.  Interestingly, a code for “vaxx” refusal is not in the 2022 version of this document, although adverse events do receive an overarching single code.

In British Columbia, ICD 9 codes are still used by the province’s medical services plan to ensure the doctor gets paid for their services.  The usage guide for submitting these codes is linked above, and details every usage, including how to ensure payee details are in the system.

I took a look at some of the codes in a few of the documents on the above site and found these interesting tidbits of info. Do note that anything in all caps was copied directly from the associated PDFs, and any misspellings are left in place, as is.

Under the heading:

SUPPLEMENTARY CLASSIFICATION OF FACTORS INFLUENCING HEATLH STATUS AND CONTACT WITH HEALTH SERVICES (V01 – V82) PERSONS WITH HEALTH HAZARDS RELATED TO COMMUNICABLE DISEASES (V01 – V07.9)

One code in ICD9 is V04, defined as “NEED FOR PROPHYLACTIC VACCINATION AND INOCULATION AGAINST CERTAIN VIRAL DISEASES”

Three other codes not far from this one are:

V05.8 OTHER SPECIFIED DISEASE
V05.9 UNSPECIFIED
V06 NEED FOR PROPHYLACTIC VACCINATION AND INOCULATION AGAINST
COMBINATIONS OF DISEASES

Under the heading:

PERSONS WITH POTENTIAL HEALTH HAZARDS RELATED TO PERSONAL AND FAMILY HISTORY (V10 – V19.9)

My allergic reaction to penicillin would have been coded as V14.0

There is a heading called:

PERSONS ENCOUNTERING HEALTH SERVICES IN OTHER CIRCUMSTANCES
(V60 – V68.9)

Where we find this entry:

V62.6 REFUSAL OF TREATMENT FOR REASONS OF RELIGION OR CONSCIENCE

This may be the closest option to track the unvaxxed that I was able to dig up for doctors who practice medicine in the province of BC, Canada.

This next entry would be useful right now if governments and regulatory bodies allowed for it:

V64.0 VACCINATION NOT CARRIED OUT BECAUSE OF CONTRAINDICATION

Did you know you get entered into the system if you merely accompany someone into the hospital???

cash registerUnfortunately, in my peeking around this section of the BC government’s website, I ran into information about carrying out MAiD, including the codes to be used so that physicians and pharmacists can be paid for killing someone who has ostensibly requested the offing. This info is at the bottom of the MAiD page here. The fee schedule provided by the government offers the doctor $283 for carrying out the murder! Other fees relate to assessing the requestor for such a service, and the obtaining of the “medication” to carry out the request.

The fee payment schedule that was immediately available was dated to 2018, but digging deeper across the BC government MSP area, eventually you DO come across updated MSC payment schedules, including “temporary” changes due to the pandemic.

According to an amendment that has been updated a few times since 2020, doctors in BC who are part of the MSP system, get paid $5 to $14 for delivery of COVID vaxx’s and “extended immunization” as they refer to boosters in the PDF. In April 2021, an advice fee was added around $17.xx. Each fee has a code. The “temporary” fees were announced January 2021, but according to Doctors of BC, followed on new fees added Fall of 2020, for respiratory immunizations.

On this page here, at the very bottom, it is possible to deal with physicians who do not get paid by MSP. They are limited to what they can charge the patient, but apparently it is possible to work as a doctor in the province of British Columbia, without having to use the MSP system to get paid. This is useful information. However, even a physician who has opted not to be part of the MSP system, CAN submit requests for payment from MSP to cover patient visits AND would then still have to use the associated codes. 

As of yet, I have not located Canadian usage of the three new “vaxx” related codes, but that doesn’t mean they may not appear here or are here under a label or form I simply haven’t dug up. It is thoroughly possible that the refusal of treatment code may get used or amended for use, to enable this. We shall have to see.
